There seems to be big problem with refinery startup proceedures these days – almost all fires last couple years have been from units starting up after a shutdown, a number of them in coking units.

I still believe most Refiners have outsourced too much to contractors who do not have good safety programs or training. Also have highest death tolls on accidents as proof.
